Note the links above are Checklist Forms that Fellows participants can fill out each month. CSLI Headquarters can email you the results of your Survey once per month. Just send a request to [email protected] and he will send you the results of your survey.

 

FOR ADVANCED USERS ONLY

If you would like to access the Results of the Surveys yourself at any time, or if you would like to customize your surveys or allow your mentors to see the results, these can be done but will require some additional steps on your part as outlined below:

 

SETTING UP YOUR GOOGLE ACCESS ACCOUNT

If you already have a Gmail or Google account, you can send an email to [email protected] using your Google email account. Shortly thereafter you'll be added as a collaborator on all of the the Fellows Checklist Google Forms. You can then proceed to the "Instructions to Review Results" section below. If you don't have a Gmail or Google Account, continue first with these steps:

Step 1: Set up your Google Access Account by clicking here.

Step 2: You can link your existing CSLI account to Google by clicking on "Use my current email address instead"

Step 3. Enter your first name and last name and your CSLI email address. Then add a password.

Step 4: Once you've set up up your Google Access Account, send an email to [email protected] and he will add you as a collaborator on all of the Fellows Checklists Google Forms.

INSTRUCTIONS TO REVIEW RESULTS

Step 1: Click here to view all of the available Fellows Checklists

Step 2: Navigate to the appropriate folder and select the Form that you would like to view:

Step 3: If you've successfully set up your Google Access Account, are logged in, and have been assigned as a Collaborator you should see an option for "Responses". Click on it.

 

Step 4: Click on the Three Dots in the Top Right and Download the Responses as a CSV File

Step 5: Open the Excel file with the responses (Note on some computers you may need to unzip the file first)

Step 6: In Excel, Click on the "Data" menu and sort the data based on the City (Column C). Then Click "OK"

Step 7: Scroll down to your City in the Spreadsheet to view your data. Any blank entries in the spreadsheet represent incomplete assignments.

GOING DEEPER WITH GOOGLE FORMS

CUSTOMIZING YOUR CITY'S CHECKLISTS AND GRANTING MENTOR ACCESS

Currently we are using one Google Form each month for all Cities. If you would like to include specific questions for your City or if you would like to share access and results with each of your mentors, it can be done. Here's how:

Step 1: Select the Google Form that you would like to customize.  Then right click on the form and select "Make a Copy"

Step 2: Rename your form so that you can find it easily.

Step 3: Now you can double click on your new form to go in and edit the titles or any of the questions. You can also add any questions that you like: short answer, multiple choice, etc. If you'd like more information on all of the ways you can customize your form, click here.

Step 4: When you're done with your edits you can share this form with your Fellows by clicking on the Send button at the top right of the screen and selecting the link icon. Simply copy and paste the link to your form to share with your Fellows.

Step 5: If you would like to grant access to form results to your Mentors, you can add them as Collaborators. Right click on the three dots in the top right of your screen and select "Add Collaborators."

Step 6: Add your mentor's email address in the form so that they can access the form results. If your Mentors do not have Google accounts you'll need to include a notification message so that they can accept access or create their own accounts.
